Location/ Where to find

• Testing Chamber
• GB&A - Complete any Testing Chamber level at least once with all four of the 3rd row of modifiers having been active.  You can't activate more than one modifier per row, so it will take at least four runs.  Once you've completed a level with the final modifier, it will be in the safe at the end.
